MENTAL HEALTH / CASE MANAGEMENT

BHC's mental health services include a range of counseling, case management and education to children, youth and adults who have been marginalized due to poverty, race, mental illness, substance abuse, HIV/AIDS and homelessness.
Our mental health providers address severe mental illness as well as depression, anxiety, substance abuse, parenting concerns, stress, among other issues.

In the integrated behavioral health program, mental health clinicians work with primary care providers as a team to treat the mental and physical health needs of their patients. Mental health case management involves assessing the needs of a patient, implementing an individually tailored care plan and coordinating community services to ensure the best possible outcomes.
